BASIC INFORMATION DESCRIPTION A serious infection or inflammation of part or all of the peritoneum, the covering of the intestinal tract. FREQUENT SIGNS AND SYMPTOMS Pain in one area or throughout the abdomen. Pain usually starts suddenly and becomes increasingly severe. Pain may be cramp-like at first, and then steady. The patient often prefers to lie quietly on the back because movement or pressure on the abdomen increases pain. Shoulder pain sometimes ; . Chills and fever often high ; Dizziness and weakness. Rapid heartbeat. Low blood pressure. CAUSES Intense inflammation of the peritoneum lining that occurs when foreign material enters the abdominal cavity. Foreign material includes bacteria or gastrointestinal contents, such as digestive juices, blood, partly digested food or feces. These material enter the abdomen following: Rupture or perforation of any organ in the abdomen, such as an inflamed appendix, peptic ulcer, or infected diverticulum or gallbladder. Injury to the abdominal wall, such as from a knife or bullet wound. Pelvic inflammatory disease. Rupture of an ectopic pregnancy. RISK INCREASES WITH Delay in treatment of causes listed above. Recent abdominal surgery. Corticosteroid therapy. Advanced liver disease. PREVENTIVE MEASURES Obtain prompt medical treatment for underlying disorders. EXPECTED OUTCOME Usually curable with early diagnosis and treatment. Treatment delay and complications can be fatal. Outcome dependent on age, duration of illness, cause, and any preexisting condition. POSSIBLE COMPLICATIONS Shock. Blood poisoning septicemia ; . Intestinal obstruction caused by later adhesions bands of scar tissue ; . Kidney or liver failure. TREATMENT GENERAL MEASURES.
I wasn't absolutely sure of what I read, so I went back, and sure enough I find this very strange. I was very focused on lithium and inositol, because of thinking it was the inositol I had a reaction to. If you remember, I had a reaction to the supplement within 15 min., so there was no doubt it was either choline, inositol, or phosphatidylserine. Without getting too much into that topic tonight, here are a couple of studies that were in my saved data, for my future posting. The therapeutic mechanisms of lithium for treating bipolar mood disorder remain poorly understood. Recent studies demonstrate that lithium has neuroprotective actions against a variety of insults. Here, we studied neuroprotective effects of lithium against excitotoxicity in cultured cerebral cortical neurons. Glutamate-induced excitotoxicity in cortical neurons was exclusively mediated by NMDA receptors. Pre-treatment of cortical neurons with LiCl time-dependently suppressed excitotoxicity with maximal protection after 6 days of pre-treatment. Significant protection was observed at the therapeutic and subtherapeutic concentration of 0.21.6 mm LiCl with almost complete protection at 1 mm. Neuroprotection was also elicited by valproate, another major mood-stabilizer. The neuroprotective effects of lithium coincided with inhibition of NMDA receptor-mediated calcium influx. Lithium pre-treatment did not alter total protein levels of NR1, NR2A and NR2B subunits of NMDA receptors. However, it did markedly reduce the level of NR2B phosphorylation at Tyr1472 and this was temporally associated with its neuroprotective effect. Because NR2B tyrosine phosphorylation has been positively correlated with NMDA receptor-mediated synaptic activity and excitotoxicity, the suppression of NR2B phosphorylation by lithium is likely to result in the inactivation of NMDA receptors and contributes to neuroprotection against excitotoxicity. This action could also be relevant to its clinical efficacy for bipolar patients. : jneurochem cgi content abstract 80 4 589 Stress elevates cortisol, which in turn ups the excitatory neurotransmitter glutamate, which increases calcium influx into the neuron and activates certain calcium-dependent "death" enzymes. Cortisol may also reduce the neuron's capacity to take energy-sustaining glucose into the cell so it doesn't have the strength to deal with a subsequent crisis. Another casualty may be the glia, the "other" brain cell, once thought of as mere mind-glue but now recognized as an active partner of the neuron. One of its functions is thought to be clearing glutamate from the synapse. Basically, Dr Manji explains, the cells can't handle the load. Their atrophy and death tends to isolate neurons, affecting their ability to connect to and communicate with other neurons. The good news is that some of the meds we are taking may actually prevent and even reverse these processes With new gene technology, Dr Manji and his colleagues started experimenting with lithium and Depaoote I believe this is valproic acid ; on brain cell tissue, and found to their surprise these two completely different medications indirectly affected some of the same cell pathways associated with cell survival and death. One protective protein that utilizes these pathways is Bcl-2, which in one experiment was doubled by lithium and Depakotte administration. Subsequent experiments on rats found lithium mitigated the effects of lab-induced stroke and led to the growth of new neurons in the hippocampus. When Dr Manji asked Dr Drevets to revisit his study, it was found that those patients on lithium or Depakot4 did not show brain atrophy. More recently, a study on human patients with bipolar found lithium increased.

Hat does a physician do when a procedure he has done by the book nearly causes a patient's death? And, what should a patient do when she nearly dies from a medical accident, but no one at the hospital wants to talk to her about it? In a dramatic keynote address at Fairview's Quality Leadership Conference in March, a physician and his patient described a devastating medical near-tragedy and explained how--through courage, compassion and forgiveness--they've been able to improve the way their medical community handles adverse events. "It has been an amazing journey, " said Fredrick van Pelt, M.D., an anesthesiologist at Brigham and Women's Hospital in Boston. "What started out as a compulsion to do the right thing for my own healing has turned into something much bigger.

Joel Stokes v. National Presto Industries, Inc., WD64128 Mo. App. W.D. 2005 ; . When thirteen month old, Joel Stokes pulled the cord of a fryer. The fryer slid off a counter and spilled hot oil on him. He was seriously injured. Stokes parents, acting on their son s behalf, sued the fryer s manufacturer, National Presto Industries, Inc. and the boy s grandmother, who was cooking with the fryer when the mishap occurred. In the pretrial phase, the trial court limited the discovery produced by National Presto to the specific accident model, a National Presto Kitchen Kettle . The plaintiff was able to discover additional accidents, involving other fryers, from the Consumer Products Safety Commission and Underwriter Laboratories. These accidents involved youngsters pulling fryers off counters and injuring themselves. At trial, the court limited the plaintiff s counsel to the three prior incidents involving the same model, the Kitchen Kettle . The jury returned a verdict for National Presto Industries and against the grandmother. The plaintiffs appealed. The Missouri Court of Appeals for the Western District held that the trial court abused its discretion in limiting the evidence to the Kitchen Kettle model. The court commented that National Presto manufactured three other units that were designed for deep frying food: the FryBaby, the FryDaddy and the GranPappy models. While these differed in capacities, they all have an aluminum pot, a cooking oil fill-line and plastic feet. However, unlike the Kitchen Kettle model, the cords are not detachable. The court of appeals did not rule that all prior incidents should be admissible. It instructed the circuit court, upon retrial, to determine whether or not the pullover incidents involving Presto s other deep fryers were substantially similar to Stokes accident. The appellate court gave some guidance as to what was sufficiently similar . To be sufficiently similar the accidents must: 1 ; be of like character, 2 ; have occurred under substantially the same circumstances, and 3 ; have resulted from the same cause. While the circuit court was instructed to evaluate the prior incidents and determine whether or not they were sufficiently similar, the appellate court also admonished the circuit court to not interpret the decision to mean that it must allow Stokes to delve into all of the details of the prior incidents, involving National Presto s other products. For instance, Stokes sought to introduce photographs of injuries, deposition testimony, allegations from complaints filed in lawsuits against Presto, and testimony from some of the victims parents. The appellate court commented that such detail could be unduly prejudicial and could cause confusion by becoming the proverbial side show taking over the circus . The appellate court also instructed the trial court to allow Stokes to discover claims involving similar incidents with National Presto s similar deep fryers. The Stokes case is instructive because it predicts the evidence allowed at trial concerning prior similar incidents. This opinion sends the message to manufacturers that plaintiffs are not limited to the specific model that was involved in the accident that is the subject matter of the lawsuit. Rather, plaintiffs are allowed to discover and or adduce evidence concerning similar incidents involving similar models.

Claimant to resolve the matter or initiate an action under the registrys Start-up Trademark Opposition Policy STOP ; . Only trademark owners who submitted an IP Claim by August 8 may access STOP. Also, only exact matches will trigger notification for example, if a trademark owner filed an IP Claim for mark and another party then filed an application for mymark , the trademark owner will not be notified. The advantage of STOP over existing dispute resolution procedures such as ICANNs Uniform Domain-Name Dispute Resolution Policy UDRP ; is that it carries a lower burden of proof. A STOP claimant can prevail by showing either use or registration of the mark in bad faith, while the UDRP requires both. Although the registry has not yet established clear rules, failure to withdraw a registration after notification of an IP Claim might be deemed evidence, in and of itself, of badfaith registration. If so, STOP could prove a quick and effective tool for trademark owners to stop cybersquatters. NeuLevel reports that approximately 50% of all IP Claims were filed by U.S.based entities. 32% of all IP Claims came from Europe, 11% from the Asia Pacific region, and 7% from other areas of the globe. Failure to Submit an IP Claim In the absence of an IP Claim, any competing applications for identical domain names will be resolved by random draw at the end of September. Then, after October 1, 2001 when goes live ; , subsequent applications will be considered on a first-come, first-served basis. Note that filing an IP Claim does not eliminate the need to file ones own domain name application; it merely entitles the IP Claimant to notification of any conflicting application filed during the landrush period, and the right to proceed un and effexor.

Papaverine infusion when compared to balloon angioplasty. Finally, because the risks of perforation are less with a microcatheter compared to a balloon, papaverine infusion has been viewed as less invasive. A well-substantiated advantage of papaverine infusion is that it can safely be coupled with balloon angioplasty 18, 19, 27, ; . Angioplasty is performed for the proximal vessels and papaverine infusion for the more distal vasospasm. In this situation, papaverine infusion is usually performed first because some believe the vasodilator makes vasospasm more responsive to balloon angioplasty 18, 57 ; . However, there are conflicting data regarding these points. Firlik et al used papaverine in eight of 14 patients as an adjunct to balloon dilation, but the papaverine had no angiographic effect in four 50% ; and only minimal angiographic effect in four 50% ; , prompting the authors to concluded it was of only marginal benefit 27 ; . The results of recent series of papaverine infusion for cerebral vasospasm are given in Table 2 51, 55 ; . A relatively good angiographic result can be obtained with papaverine infusion, ranging from 67% to 98% for these studies Fig 4 ; . Modest degrees of weight gain can occur on carbamazepine and gabapentin, with more prominent weight gain in patients on valproate Depakote, Depakene ; , in parallel with weight gain on lithium see table, p. 9 ; . Despite promising open trials with adjunctive topiramate, four recent, large, controlled studies of topiramate in acute mania indicate that it is not an acute antimanic agent. Nonetheless, it may have utility in assisting with weight loss. The drug may also be particularly useful in post traumatic stress disorder Berlant 2001; J Clin Psychiatry 62 [Suppl. 17]: 6063 ; and in helping to decrease alcohol abuse Johnson et al., 2003; Lancet 361 [9370]: 16771685 ; . In a recent randomized study in the SFBN, topiramate and sibutramine Meridia , a weight-loss drug ; showed parallel degrees of weight loss Frye et al., 2003 ; . This weight loss averaged about a third of a pound per week, very similar to the weight loss achieved by zonisamide. Whether adjunctive topiramate will emerge as more of a mood stabilizer than sibutramine which was originally developed as an antidepressant ; remains to be examined in the next round of data analysis. Dr. C. Normann University of Freiburg, Germany ; presented data from a clinical trial in mainly unipolar patients of lamotrigine versus placebo. All patients received paroxetine Paxil ; as well. Lamotrigine had a more rapid onset of antidepressant effects in unipolar depression than in those patients on placebo. Dr. G. Goodwin University of Oxford, UK ; reviewed the literature on treatment of bipolar depression and emphasized the great lack of data, par Continued on page 9.
